How the Plan Works
You can enroll in Critical Illness Insurance administered by Lincoln Financial. The diagnosis of a critical illness can be devastating, and although you may have health and disability coverage, there are many expenses associated with your treatment (such as travel, child care and other unexpected costs) that you may not have considered.
Critical Illness Insurance coverage provides a lump-sum cash benefit to help you cover the out-of-pocket expenses associated with the following illnesses:
- Heart attack
- Stroke
- Invasive cancer
- Renal (kidney) failure
- Major organ failure
- Additional childhood conditions
- Arterial/vascular disease
- Noninvasive cancer (in situ)
- Advanced Alzheimer’s disease
- Advanced multiple sclerosis
- Benign brain tumor
You can choose from four benefit levels—$5,000, $10,000, $15,000 and $20,000. Additional coverage options for your spouse and/or children are available if you choose to cover yourself.
You are not required to provide evidence of insurability (medical underwriting) if you elect Critical Illness Insurance coverage during your new hire enrollment period. If you decline coverage during this enrollment period and decide to enroll at a later time, you will be subject to medical underwriting. Premiums are determined by your age and smoker status. When you enroll in the ZONE, you will see the rates specific to your age and smoker status.
