Company-Paid Coverage

Group Life and AD&D

Zebra provides basic life and Accidental Death and Dismemberment (AD&D) insurance at no cost to you. The benefit is 1.5 times your annual salary, up to $1,000,000. The cost of coverage for basic life insurance above $50,000 will be included as imputed income for tax purposes. Be sure to enter your life insurance beneficiary information on the ZONE when you enroll.

Dependent Life Insurance

Zebra provides life insurance for your spouse/domestic partner and dependent children at no cost to you. Spouses/domestic partners receive $5,000 in coverage and dependent children receive $1,000 in coverage. Although you do not need to actively enroll in dependent life insurance, you do need to add your spouse/domestic partner and/or dependent children as dependents on the ZONE in order to receive this benefit. You are the beneficiary for the dependent life insurance policies. 

Voluntary Life and AD&D

Employee Voluntary Life Insurance

You can enroll in additional life insurance in increments of $10,000, up to 5x your annual base salary or $1,000,000, whichever is less. Rates are determined by your age and smoker status. Evidence of insurability (EOI) will apply for any coverage amounts over $300,000. If you choose not to enroll in this benefit during your new hire enrollment window, you will be required to complete EOI should you elect any coverage amount in a future enrollment period. EOI is a medical questionnaire used to determine proof of good health.

Spouse/Domestic Partner and Child Voluntary Life

You can also enroll in additional life insurance for your spouse/domestic partner and/or dependent children. To enroll your dependent children, you or your spouse/domestic partner must be approved for Voluntary Life coverage.

You may elect up to $500,000 (in $10,000 increments) for your spouse/domestic partner. The Voluntary Life coverage for your spouse/domestic partner cannot exceed the amount elected for Voluntary Employee Life.

If you did not enroll in this benefit during your new hire enrollment window and want to enroll now, you will be required to complete EOI. If you are currently enrolled in the plan and you choose to increase your coverage amount during annual enrollment, you will be required to complete EOI.

You may elect up to $10,000 (in $2,500 increments) for each of your dependent children. Child Voluntary Life is not subject to EOI.

Employee, Spouse/Domestic Partner and Child Voluntary AD&D Insurance

You can enroll in additional AD&D insurance for yourself, your spouse/domestic partner and dependent children. Employee coverage can be purchased in increments of $10,000, up to 5x your annual base salary or $1,000,000, whichever is less.

Spouse/domestic partner coverage can be purchased in increments of $10,000, up to $500,000. Dependent child coverage can be purchased in increments of $2,500, up to $10,000. The combination of spouse/domestic partner and child coverage cannot exceed your election amount. EOI is not required.

Evidence of Insurability

If you elected any coverage amounts that require EOI, Lincoln Financial will review your health status before approving your coverage. In the future, if you would like to increase your coverage amount, you will be required to submit EOI. Go to My Lincoln Portal at www.mylincolnportal.com to access the online EOI form.
