Employee Eligibility
You are eligible for benefits if you work at least 20 hours per week. Benefits begin on your first day of employment. New hires have 30 days to enroll in benefits.
Dependent Eligibility
Coverage for your dependents is available under the Zebra benefits program, provided they meet all eligibility requirements.
Your eligible dependents include:
- Your spouse, unless you are legally separated. Your spouse must be:
- Recognized as your spouse for purposes of federal tax law; and
- A U.S. citizen or national or a resident of the U.S., Mexico or Canada.
- Your same-sex or opposite-sex domestic partner.
- Your dependent children who have not reached age 26. Children include the following individuals:
- Your natural and legally adopted children;
- Any child over whom you have legal guardianship; or
- A child of your spouse or domestic partner.
As long as your coverage continues, your disabled child may remain covered to any age if they are unmarried and are determined by the Social Security Administration to be disabled as of age 26. Refer to your Summary Plan Description for more details about dependent eligibility.
Verification of Eligibility
Dependent coverage begins once you submit your elections. However, within 30 days from the date you complete your enrollment, you must submit documentation to ADP to verify the eligibility of your dependent(s).
ADP will notify you soon after you enroll with details about how to provide proof of dependent eligibility. If you fail to provide documentation that proves your dependent is eligible, coverage for that dependent will be dropped.
To assist you in getting a jump start on finding the documents you will need, here are examples of the required documentation for your spouse or domestic partner:
- A marriage certificate or affidavit of domestic partnership AND one of the following dated within the last six months from adding your spouse or domestic partner to coverage:
- Bank or credit card statement with common address
- Mortgage or lease statement with common address
- Motor vehicle statement with common address
- Utility bill with common address
Examples of required documentation for your child(ren) include one of the following:
- Adoption certificate
- Birth certificate with parent’s name listed
- Documentation of legal custody or guardianship
- Hospital birth record (within 90 days of birth)
Once a dependent has been certified, you will never be asked to certify the dependent again.

Important Enrollment Reminders
Dependent SSNs are required—The IRS requires you to provide a valid Social Security Number (SSN) for yourself and any dependents (spouse/domestic partner and/or children) covered by a Zebra health plan. This is a requirement of the Affordable Care Act, and Zebra will be required to report this information to the IRS to demonstrate that the company has made minimum essential coverage available to eligible employees. You may enter any missing SSNs when you go online to the ZONE to enroll and manage your dependents.
Dependent life insurance—Zebra automatically provides life insurance coverage for your spouse/domestic partner ($5,000) and dependent children ($1,000 each) at no cost to you. You do not need to actively enroll for this coverage, but you do need to list your spouse/domestic partner and children as dependents on the ZONE to receive this benefit.
