Choose from Two Dental Plans

Zebra offers dental insurance through Delta Dental, the largest dental benefits carrier in the country. You may choose from two dental plan options—the Advantage Plan and the Basic Plan—each offering two networks with different coverage levels.

With both the PPO network and the Premier network, you can choose any in-network provider and pay less out of pocket. However, if you choose an out-of-network provider, you will pay more out of pocket and you may be billed the difference between what your provider charges and what Delta Dental will reimburse.

No-Cost Employee Coverage – Employee only coverage in the Basic Dental Plan is available with no payroll deductions. The Basic Dental Plan has the same dental networks and similar benefits to the Advantage Dental Plan. It has a $1,500 maximum annual benefit but does not cover dental implants or orthodontia.

  • Emergency exams and palliative treatments

  • Fillings

Dental Networks

Both dental plans are comprised of three networks – PPO Network, Premier Network, and Out-of-Network. Dentists in the PPO network will charge less for services than dentists in the Premier network, while Out-of-Network dentists are able to charge the most for services.

Delta Dental PPO

  • Standard provider network.
  • Lower out-of-pocket expenses for members.
  • PPO dentists may not bill you for charges exceeding the contracted fee.

Delta Dental Premier

  • Expanded provider network.
  • Higher out-of-pocket expenses for members.
  • Premier dentists may not bill you for charges exceeding the contracted fee.

Out-of-Network Dentists

  • No discounts.
  • Out-of-network dentists may balance bill you for charges in excess of Delta Dental’s reimbursement.

ID Cards

If you are enrolling in a Zebra dental plan for the first time, you will receive a new dental ID card mailed to your home.
