Medical Plans
You can choose from three medical plans with BlueCross BlueShield of Illinois. Two of the plans allow you to save money in a Health Savings Account (HSA).
Our three major medical plans (Basic HSA, Advantage HSA and Advantage PPO) are administered by BlueCross Blue Shield (BCBS) of Illinois. All three plans cover the same services and include the same in-network providers and facilities.
No matter which plan you choose, you will get:
- Free preventive care when you use in-network providers
- Access to the BCBS Participating Provider Organization network in most states (in Wisconsin, it is the Blue Preferred POS network; in Georgia, it is Blue Open POS network)
- The option for no-cost employee coverage when you elect the Basic HSA plan and cover only yourself (no dependents); complete the required Well Track activities annually to earn the $600 premium reduction (not required for new hires)
- Inclusive coverage, including transgender benefits, IVF (up to two cycles) and IUI, ABA therapy for the treatment of Autism Spectrum Disorder, and reimbursement of travel expenses to receive healthcare
If you choose the Basic HSA or Advantage HSA plan, a Health Savings Account (HSA) will be opened for you through HSA Bank. Learn more about Health Savings Accounts.

*If costs for one family member reach the individual out-of-pocket maximum allowed by the plan before the family out-of-pocket maximum has been met, the plan will begin to pay 100% of costs for that individual.
Key Terms
Copay: A fixed amount you pay for certain healthcare services at the time you receive them
Annual Deductible: The amount you pay before coinsurance begins (when the plan shares the cost of healthcare services)
- Basic HSA and Advantage HSA: Family deductible; coinsurance begins when the family’s combined expenses reach the deductible amount
- Advantage PPO: Individual deductibles; coinsurance begins for each individual (up to three) when his or her expenses reach the deductible amount
Out-of-Pocket Expense Limit: The most you’ll pay for eligible healthcare expenses in a year before the plan pays 100% of eligible expenses
- Basic HSA and Advantage HSA: Includes medical/prescription drug deductible and coinsurance payments. The plan pays 100% of costs after one family member reaches the limit
- Advantage PPO: Includes medical deductible, copays and coinsurance. The plan pays 100% of costs for each person (up to three) when he or she reaches the limit
Reimbursement of Travel Expenses to Receive Healthcare
If you are enrolled in a Zebra BCBS medical plan and do not have access to an in-network doctor or facility within 100 miles of your home, the medical plan will reimburse eligible transportation and lodging expenses for travel that is necessary to obtain covered services.
- Transportation: Round trip air, train, bus, taxi/ridesharing services, rental car, tolls, gas, and parking; mileage reimbursement reflects the IRS medical mileage reimbursement rate (20.5 cents per mile in 2026)
- Lodging: Up to $50 per night for the patient, plus an additional $50 per night for one companion (two companions if the patient is under age 18)
Travel benefits are limited to a maximum of $4,000 annually. You will need to submit receipts to BlueCross BlueShield for reimbursement.
ID Cards
When you enroll in a Zebra BCBS medical plan for the first time or change medical plans, you will receive a medical ID card from BCBS and a prescription ID card from Optum Rx in the mail at your home address. You may also access your ID cards through the BCBS and Optum Rx mobile apps.
